In practice the largest sustainable savings come from removing waste, variation and rework that also harm patient experience — pathway redesign, reducing length of stay through better discharge planning, standardising procurement, and eliminating duplicated administrative effort across organisational boundaries.

We build clinical leadership into governance from the start — a named clinical lead with genuine decision rights, forums timed around clinical commitments, and engagement that treats frontline scepticism as information rather than resistance.
The same applies to system working. Integrated Care Board priorities, provider collaboratives and place-based partnerships each bring their own governance; we design programme structures that respect existing accountabilities rather than adding another layer on top.
